Insurance is filed for you free of charge for office visits, hospital admission or surgery and is mailed directly to your insurance company or to your employer if it is a group policy.

Provide accurate and complete insurance information to present proper documentation. Please bring all insurance cards to show to the receptionist; these will be copied and placed in your medical records. It is your responsiblity to let us know of any special arrangements required of your carrier, such as second opinions, pre-admission, pre-certification or any change in your coverage.

Our office manager is on staff to assist you with any questions or problems that you may have involving your insurance needs. We do participate in most insurance plans. Some of which include:

Please call your carrier to confirm your insurance coverage, if you are unsure that they participate in our network.
